I suggest
printing these questions out and bringing this list with you to
the agency, when you go for your non-identifying information.
This way you don't forget anything. Also, if the caseworker
leaves you in the room with your record on the desk. LOOK AT
IT!! Social Workers often do this, they want you to look at,
because legally they cannot give you any identifying information.
(This is sometimes their way of getting around this law)

Could you contact the foster parents? (I would like to meet
them?)
What was the date of final adoption?
What was the court of jurisdiction?
Who was the delivering Doctor?
Who was the pediatrician that cared for me?
Were there any lawyer involved?
Did either of my birthparents have any medical problems?
Did my biological grandparents have any medical problems?

Of what
descent was my birthmother?
What religion was my birthmother?
How tall was my birthmother?
What did my birthmother look like?
Did she have any other children?
Can you tell me their names? (at least first name?)
When were they born?
Where were they born?
What was my birthmothers first name?
What was my birthmothers last initial?
What was my birthmothers date of birth? (at least year?)

ADOPTEE WAIVER, when you go for your non-identifying info. The
agency cannot release your name or whereabouts without this
waiver in place!
