Personal Questionnaire for the Dakota Icelanders Survey
The Personal Questionnaire for the Dakota Icelanders Survey is a one-page brochure with information about the survey and contact information on one side and the actual questionnaire on the other side that can be folded to standard envelope size and mailed. Below are two links that will provide Adobe Acrobat (extension pdf) files for the two sides of the survey form. Most computers have the ability to print such files directly.

Of course, it is not necessary that the forms be used. Information can be submitted in any manner that is convenient. If you have existing documents of any type relating to the Dakota Icelanders, copies of them can be submitted. Or the information can be submitted by email. Merely copy and paste the items listed below and provide the information as listed in an email and send it to arlans@aol.com

Who should participate?
All descendents of people who lived for any time in the Dakota settlement.
What is The Dakota Icelanders Project?
The goal is to create a complete and accurate historical record of the Icelandic settlements in North Dakota.
How can you participate?
Provide information as described in this brochure and let others know about the project.
If you need several forms please duplicate the form on the reverse side as necessary.
Keep the Icelandic spirit alive!
The Dakota Icelanders Survey
Inside is a form that will guide you in providing information for the survey that is the most important part of the Dakota Icelanders Project. It is not necessary that all items be filled in, but the more information provided the more valuable the form is.
Please fill in one form for each person to be included in the survey. For instance, fill out one for yourself, one for your spouse (if married), one for each adult child in your family, one for each living ancestor (you may wish to give them a form to fill in).
Most important fill out a form for each deceased ancestor that you may have information about and provide forms to anyone else you think may wish to participate in the survey.
When finished, fold so the mailing address is on the outside, add a return address and stamp, and mail (or submit several forms together).

If you would prefer to use the internet for submitting information simply send an email with the relevant information to arlans@aol.com or request a text version of the questionnaire. Note that the email questionnaire is considerably more comprehensive.
