• Dr. Liberty and Dr. Kovar are both board certified Pediatricians and members of the American Academy of Pediatrics

• We will offer routine check-ups including immunizations as well as same day sick appointments

• Pediatric RNs work during normal office hours and can answer many of your questions. We also have a web-site that covers many common pediatric concerns. This can be accessed at: www.childrensmemorial.org/pal.

• When we are not in the office, one of the physicians will be on call to answer urgent medical questions.

• Kids First Pediatrics will be state of the art. Some of the features will include:

            The use of Electronic Medical Records through a secure HIPPA compliant connection (128 bit encryption):

• Allows the physicians to access patient records even after hours and on week-ends
• Gives accurate percentiles for the growth parameters
• Prescriptions are legible reducing medication errors and can be faxed to the pharmacy of your choice from the office so you can pick it up on the way home (Please note that the Pharmacy has to accept fax copies of prescriptions)
• The ability to check your child’s most recent clinical information (immunizations, weight, height, etc) online anytime
• The ability to see your child’s currently prescribed medications (including dosing) and request refills of chronic medications (such as allergy or asthma meds) online from your home
• The ability to request school physicals online and have them sent to you in printable PDF format
• The ability to pre-register online eliminating the need to fill out paperwork when you come to the office
• The ability to see any account balances online

CPR for Family and Friends

The Family & Friends CPR program teaches how to perform CPR in adults, children, or infants and how to help an adult, child, or infant who is choking. This program is designed for family members, friends and members of the general community who want to learn CPR but do not need a certification card. A participation card is offered at the end of the session.

This course is taught by Jennifer Fritsch, R.N., a CPR instructor certified by the American Heart Association. The course is offered at our office on the following days:

Saturday August 25, 2007 @ 12:30 p.m.

Saturday September 22, 2007 @ 12:30 p.m.

The cost for this course is $30 per person or $50 per couple. This cost includes a Family & Friends CPR Student Book per participant as well as an adult, infant & child CPR reminder cards.

Classes are limited to 6 people per class. The fee is non-refundable and must be paid at the time of registration. A registration may be moved to another class time, if space is available, however, this can only be accommodated one time.

Course time is approximately 2˝ hours.

-         This is a form to release medical records TO  Kids First Pediatrics

-         Please fill in the information and forward it to your former provider.

-         Please note that it may take a few weeks from the time that your former provider receives the request until we receive the records.

-         Your former provider may charge a fee for photocopying the records

Release of Medical Records

-         This is a form to release medical records FROM  Kids First Pediatrics

-         Please be aware that there is a $20 fee* for copies of medical records from Kids First Pediatrics

                                                                                             *This fee is waived if only immunization records are requested

HIPPA form

Authorization to treat a minor

-         Please fill the form out and fax it to our office at (815) 609-8111 if someone other than the parent or guardian will bring the child into the office

-         Please note that a parent or guardian must be present for immunizations to be given. We cannot accept this form for well-child visits.

Kristine Liberty, M.D. – Graduated summa cum laude in Biology and Chemistry from St. Norbert College. She obtained her medical degree from Northwestern University Medical School and completed her residency at Children’s Memorial Hospital. Dr. Liberty is board certified in Pediatrics and is a fellow of the American Academy of Pediatrics.

Steven Kovar, M.D. – Attended the University of Chicago where he earned his B.A in Biology with Special Honors. He graduated from the University of Illinois at Chicago Medical School and completed his residency at Hope Children’s Hospital in Oak Lawn, Illinois. Dr. Kovar is Board Certified in Pediatrics and is a fellow of the American Academy of Pediatrics.
